All surgeries require a $500 non-refundable deposit to be paid before the surgery date is scheduled. Once the $500 surgery deposit has been paid, no refund can be issued under any circumstances. A minimum of 50% of the balance of your surgery must be received by our office no less than 2 weeks (14 days) prior to your procedure date. If at least 50% of your surgery is not paid for at least 2 weeks prior to your procedure, your surgery will be canceled. The $500 surgery deposit will NOT be refunded. The remaining balance of your surgery must be received no less than 1 week (7 days) prior to your procedure date. If your remaining balance is not paid in full 1 week before your surgery, your surgery will be canceled.
If you choose to move the date of your surgery, this must be done no less than 2 weeks prior to the scheduled date. Any surgery moved in 2 weeks or less of the scheduled date will result in an additional $500 deposit being charged to re-schedule. If you choose to cancel your surgery, this must be done 72 hours before your scheduled date. If not canceled within this time, there will be NO refund.
